A carer can be anyone of any age who provides unpaid support (excluding carer’s allowance) to a neighbour, friend or family member who could not manage without this help, due to illness, disability, frailty, mental ill health or substance misuse.

(as a parent looking after your child, you are a “carer” if your child has any additional needs)


We are keen to identify our patients who are carers and support them wherever we can. We can help you get access to services available in the wider community as well as other support agencies.

If you are interested in finding out more, please ask reception to speak to the Carers Lead at the practice.

Allow someone to discuss your records

If you’d like to let someone you trust talk to us about your medical records on your behalf, please ask reception for a form.

By law our staff can’t discuss your medical records with carers, relatives, or friends without your permission.

If you need someone to help you, and they might need to talk to us about your health, please speak to reception for the consent form.

Blue Badge Scheme

If you’re disabled or have a health condition that affects your mobility, you can apply for a Blue Badge. 
You can also apply for a badge if you care for a child who’s disabled or has a health condition that affects their mobility.
